Your First Office Visit

Dr. Yelverton sees patients by appointment only. Patients are seen on Monday and Thursday mornings between 9:00 am and 12:30 pm.

A referral may be required by your insurance company (list of acceptable insurance carriers) in order for them to pay for your visit. Please check with your insurance company and obtain a referral from your primary physician if you need one prior to scheduling an appointment. When scheduling please indicate why you have been referred to see the doctor.

You will also need to provide your insurance information when you call to make your appointment. Bring a copy of your referral with you to your appointment as it may not arrive in time from your primary physician’s office. Also bring your insurance card and a photo ID with you. You will have to reschedule your appointment if you do not have the required referral.

You will be completing a comprehensive information package at your first office visit. This package is available as a PDF download from this site which you may complete either in print or on computer prior to coming to the office. Completing the forms ahead of time will expedite your visit. The forms may also be signed and returned by e-mail before your visit or they may be returned by fax. If you choose to complete the forms at the time of your visit please arrive 15 minutes early to allow sufficient time prior to your appointment.

If you have radiologic studies (x-ray, CT, mammogram, ultrasound, MRI) pertinent to your diagnosis you must bring both the films and the reports to your appointment. If you have laboratory and test pathology results please bring those as well. Dr. Yelverton can review imaging studies performed within the HCA hospital system at Westside Regional Medical Center or Plantation General Hospital and in the Memorial Healthcare system online. Studies performed at all other facilities should be brought to the office on CD along with written copies of the reports. If your treatment requires review of these studies and you do not have them at the time of your visit your appointment will be rescheduled.

Arrangements can be made Monday through Friday for patients who need to be seen urgently. If you need to be seen emergently please go to the nearest Emergency Room.

Diagnostic Procedures

If you have been referred for a diagnostic biopsy your first visit will consist of a consultation only. After your interview, examination and review of your films a date and location for your biopsy will be determined. Some biopsy procedures may be performed at the time of the initial visit but others are inappropriate for the office setting. Frequently diagnostic or therapeutic procedures may not be allowed at the time of the initial visit by your insurance company. Regardless of the setting, the procedure itself takes approximately 30 to 45 minutes to perform. Dr. Yelverton will discuss your options with you and select the most appropriate procedure, timing and location based upon your needs. Your biopsy results will be discussed with you at a subsequent office visit regardless of the outcome. Be aware that additional procedures or studies may be needed depending upon your results.
